Referrals

At WALKS Wound Centers we welcome referrals from primary care physicians, surgeons and specialists, nurse practitioners, physician’s assistants, podiatrists, case managers, extended care facilities and patients

Our promise to our referral sources is to provide the following in order to keep you updated/informed about the care/progress of your patient:

• Monthly patient progress reports with wound area graph and photographs
• Online link to access your patient’s record(s)
• Regular communication regarding the status of your patient
• Communicate with patient’s insurance company to obtain authorization
• Frequent outpatient clinic visits (weekly or more often, determined by the severity of each case)
• Patient education that engages patients to become active participants in proper foot care and prevention
